PARENT FORMS 2023-2024 School Year:
All these forms are overdue at this time. Please make sure that you have completed them all. If you have any questions you can reach out to anyone in the main office:
1. Emergency Card-Every child every year must fill out a blue card. They are to be filled out yearly. If you have not done so already please make sure you have filled out an emergency card as soon as possible.
2. NYC Schools Acct: Emergency Card -Please make sure the guardians on your NYC Schools account matches your Emergency Blue Card.
3. Media Consent Form-To give permission to allow us to photograph your child and use pictures for educational purposes only. We have a lot of amazing school events and we would like to share with our families. Your child's great work cannot be shared without this being complete. Complete on the NYC Schools Account.
4. Digital Equity Family Questionnaire- Share important information about your students’ access to devices and internet at home. Please complete on your child's NYC Schools Account.
5. Medical Forms/Immunization Forms- Please return to the main office. This form is for 6th Graders only.
