PAPERWORK THAT NEEDS TO BE COMPLETE:
EMERGENCY CARD (Will be given 1st day of school)-Every child every year must fill out a blue card. It is most important this gets completed as soon as possible. They are available on your child’s grade level google classroom.
NYC Schools Acct: EMERGENCY CARD -Please make sure the guardians on your NYC Schools account matches your Emergency Blue Card.
MEDICAL FORM(Send in 1st day of school)-for 6th Grade students only. Due September 8.
FAMILY INQUIRY FORM(LUNCH FORMS- All families are asked to complete the family inquiry forms. Website will also be posted on the grade level google classroom. https://www.myschoolapps.com/Home/PickDistrict
MEDIA CONSENT FORM(Will be due by end of September)-To give permission to allow us to photograph your child and use pictures for educational purposes only. We have a lot of amazing school events and we would like to share with our families. Complete on the NYC Schools Account.
MYSCHOOLS(Will announce when it is available)- 8th Grade Parents Only-HS Applications
ABSENTEE FORM- If your child is going to be absent please kindly fill out the absentee form. You do not need to call the school. You can also upload doctors notes & covid results.

Dear Parents and Guardians,
Welcome to the 2022-2023 school year. I am excited to be working with you alongside our incredible staff at IS228. I am confident that this school year will be filled with exciting opportunities for you and your child. Together, we will foster a culture of learning that is both engaging and rigorous.
